In order for your child to participate in any sport in the South Plainfield School District they must have a physical examination done within 365 days of the first practice of the season. The state mandated pre-participation physical evaluation (PPE) form is the only accepted form and must be completed in full by the examining healthcare provider and parent. If the physical examination was conducted 90 or more days prior to the first practice and a PPE form is already on file in the health office then a Health History Update Questionnaire (HHQ) must be completed and signed by the parent/guardian.
**Please note that the pre-participation physical evaluation must be completed by a Healthcare Provider who has completed the Student Athlete Cardiac Assessment Professional Development Module. Their signature with the date of completion of this module is required on the bottom of page 4 of the PPE packet.
Any forms not completed properly or signed in all the required areas by the physician, parent and student will be returned to the student, delaying participation of the athlete.
According to N.J.A.C. 6A:16-2.2(h)1iv, all PPE forms must be reviewed and approved by the school physician before a student is eligible to participate.
PPE forms or HHQ forms for HS and MS athletes are due to be submitted to the High School by July 19, 2021 to ensure all athletes are cleared and ready to play. There will be a mailbox set up on the circle side of building in the High School vestibule. Please have the forms in an enveloped labeled with the athletes name and place them in the container
Late Registration: A student who does not submit all required forms by the due date should expect a delay in the processing of paperwork and clearance for sports participation. A minimum of 72 hours for processing will be required. PPE forms submitted on the first day of tryouts/practice will not be cleared for participation until at least the third day of practice.
The PPE form and HHQ form are available on the High School or Middle School website and also on the Athletic website. All additional paperwork can be accessed on the Genesis Parent Portal.
