Middle School Sports FAQ’s

What sports are available at the Middle School?

  • Fall Sports: Field hockey, football, cross country, boys soccer, girls soccer

  • Winter Sports: Wrestling, cheer, boys basketball, girls basketball

  • Spring: Baseball, softball, track

When do the different Sports start at the Middle School?

  • Each year NJSIAA (New Jersey State Interscholastic Association) determines the dates each sports season will start. Check the Athletics page at spboe.org for updates

What medical forms are required?

  • Students registering for Athletics must have a CURRENT  Preparticipation Physical Evaluation packet on file in the health office before the student can be authorized to participate in practice/tryouts.  Click HERE for the packet.  Physicals are valid for 1 year from date of the exam and must be CURRENT as of the first day of practice/tryouts.

  • If the student’s physical exam (which is already on file in the health office)  was performed more than 90 days before the start of the sport, a Health History Update Questionnaire (HHQ) must be submitted.  Click HERE for the HHQ.

When are the Middle School sports forms DUE? 

  • Due dates are determined by the school nurse each sports season. In general, the forms are due 3-4 weeks before the start date of the sport.

Where do I submit Middle School sports forms? 

  • Submit all medical forms directly to Nurse Wollman OR to the Middle School Main Office  

  • Hard copies are preferred, however,  completed and signed medical forms can be scanned and  emailed to mmccarthy@spboe.org or faxed to 908-777-7802.

What happens if I turn in incomplete forms or turn in my forms after the due date?

  • Any forms which are incomplete OR are handed in AFTER DUE DATE risk a delay in processing and student may not be able to start the sport on the first day of practice. 

My child will be starting as a 7th grader at the Middle School this fall and is interested in a fall sport.  Who do I contact with questions?  What forms are needed?

  • All incoming 7th graders who wish to participate in a Middle School sport will need to submit the Preparticipation Physical Evaluation packet, being that this will be their first school sponsored sport.  See above “What medical forms are required?” for link and information. 

My child will be in the high school next year.  Who do I contact with questions?  Where do I hand in the forms?

  • All forms for students going into the high school should be submitted TO the High School.
