At this time all requests for transcripts must include an acceptable email for the college/university to receive your transcript. If they cannot provide that please indicate that on the form.

Student transcripts can be obtained in the SPHS School Counseling Office for any individual who no longer attends South Plainfield High School. Students 18 years and older must request their own transcripts. Please print, fill out and mail, fax or email the student records request form to the SPHS Counseling office.
